Patient Referral

One of our goals is to make the referral process quick and easy. Our admissions department works closely with a patient’s physician as well as the other healthcare providers within the continuum of care to make the patient’s transition to the LTACH environment a smooth process. Lane Frost Health and Rehabilitation Center’s team of clinical liaisons will provide an on-site evaluation of the patient as quickly as possible to determine if they meet the criteria for our unique level of care. We know that time is valuable and every minute counts when treatment is needed.

Anyone can make a referral, but a physician’s order is required for a pre-admission evaluation of a potential patient that is located in another facility, unless the patient or medical power of attorney signs a release for health information. Patients who are currently at home do not require a physician's order for an evaluation. The pre-admission evaluation is provided at no cost and we will coordinate insurance verification and pre-certificationan as required.
