The patient has significant complex medical problems necessitating extended care in an acute care setting, with round the clock nursing care, daily physician visits, aggressive rehabilitative services, and proactive case mangement and social services.

Some examples of these interventions are:

 Parenteral/enteral services
 Wound Care/Hyperbarics
 Medication Management
 IV antibiotics
 Respiratory care
 Peritoneal Dialysis
 Low tolerance rehabilitation
 Terminal Care (End of Life Issues)

Types of diagnoses include, but are not limited to:

 Decubitus ulcers and non-healing surgical/traumatic wounds

 Cardiovascular Disease
 Diseases and other conditions affecting the lungs, such as COPD or pneumonia

 Diseases of the brain and/or spinal cord, polyneuropathy
 Diseases of the Kidney, such as end-stage renal disease
 Diseases of the neuro/muscular system
 Decubitus ulcers and non-healing surgical/traumatic wounds
 Skin Disorders
 Neoplasms
 Infectious Diseases
 Major Multiple Trauma
